That's exactly where I come in. I'm Jessica Kowal, RN — a geriatric care manager who sits with your family, holds all the moving pieces, and helps you build a plan that works in real life.

My first job is to actually listen to you.

Most families find me after they’ve already talked to a dozen professionals who each saw one tiny slice of the picture. You’re repeating the same story, getting conflicting advice, and no one seems to be holding the whole situation in their head at once. My work is to sit with you, hear the real story, and then help you build a plan that matches real life—not a fantasy version of your family.

I’ve spent the last decade as a nurse in hospital case management, home health and hospice, skilled nursing, and geriatric care consulting. I’ve sat through the rushed hallway updates and the chaotic discharge days, but I’ve also been at kitchen tables where caregivers finally say what they’re actually afraid of. That’s where I do my best work.

I specialize in supporting the adult and geriatric population managing chronic diseases, as well as spouses and adult children caring for loved ones with dementia and complex medical needs. I listen for the things you’re not sure you’re “allowed” to say, translate the medical piece into plain language, and help you see how each decision will affect safety, finances, and your own capacity over time. I’m direct, but I’m not harsh. You get honesty and a plan, not judgment.
